You can use your card like a credit/debit card at any location where the primary business category is dental care, prescription eyewear, fitness/gym memberships (including approved online programs and online WW® memberships), and/or prescription hearing aids. For example, if you need a filling, you can use your Benefit Bank to cover your copay with an in-network dental provider or use it to pay for services from an out-of-network dental provider.
You can also use it to pay bills received from approved vendors (e.g., dental care, eyewear, etc.). Just call the service provider and give them your Benefit Bank card number or write your Benefit Bank card number on the billing statement.
Don’t forget! Members of our Orange, Green, and Saver No Rx plans get extra money on the Benefit Bank card that can be used to buy health care products at specific retailers or through our online partner, Medline. With Medline, you can make purchases online, by phone, or through their mail-order catalog.
Dental care
The Benefit Bank can be used for comprehensive services like a filling, oral surgery, or root canal. It can also be used to pay for preventive dental care or comprehensive services from out-of-network providers. (Fallon Medicare Plus plans with dental care include $0 copays for preventive services at in-network providers.) Services and care must be provided at a business whose primary function is providing dental services. Care must be provided by a licensed dentist, dental surgeon, endodontist, oral pathologist, periodontist, or prosthodontist.
Exclusions include: consultation fees for non-covered services and cosmetic services and items, like orthodontia, teeth whitening, Invisalign®, or mouth guards.
Eyewear
The Benefit Bank be used for prescription eyeglasses (frames and lenses), prescribed lenses only, frames only, an upgrade to existing glasses, or prescription contact lenses.
Eyewear must be purchased from a licensed optometrist or ophthalmologist’s office, or a store that focuses on eye care—like those that provide exams, write prescriptions for and sell eyeglasses.
Exclusions include: non-prescription lenses and/or contact lenses, non-prescription sunglasses, low vision aids, and store promotions or coupons.
Fitness/gym memberships
The Benefit Bank can be used for gym and fitness club memberships, as long as the facility's primary business is a full-service health club. To be eligible, a fitness facility must have cardiovascular equipment, strength-training equipment, and accommodations for exercising and improving physical fitness. These include facilities such as health clubs, physical fitness centers, athletic clubs, racquetball clubs, and tennis clubs.
The Benefit Bank can also be used for approved online fitness program services or a WW® online membership. Qualifying online fitness programs provide classes that have cardiovascular and strength training for exercising and improving fitness. Here are some examples of online fitness programs that qualify:
Exclusions include: recreational clubs, rod and gun clubs, country clubs, social clubs, public and private golf courses, ski lift tickets, lodging, meals, vitamins and supplements, physical therapy locations, personal trainers, home fitness equipment or equipment rentals (including replacement parts, air walkers, bike stands, treadmills, weight benches, free weights, home gyms, rowing machines, stair climbing machines, stationary cycles, muscle-specific resistance equipment (including abdominal rollers, thigh or buttock machines), and total body weight resistance machines), recreational equipment (including bicycles, bowling balls, game balls, golf clubs, rollerblades, skates, skis, and tennis racquets), trampolines for indoor or outdoor use, Pilates ball or bands, yoga/exercise mats, exercise videos, spa services (including massage therapy), association memberships, fees (including warrantee, cancellation, and green fees), WW meals, and fitness trackers, clothing, and shoes.
Hearing aids
The Benefit Bank can be used for prescription hearing aids that go inside or outside of the ear. The eligible device types include:
- Behind-The-Ear (BTE)
- Receiver-In-Canal (RIC)
- In-The-Ear (ITE)
- In-The-Canal (ITC)
- Completely-In-Canal (CIC)
- Invisible (IIC)
- Open Fit models
Hearing aid devices must be a new purchase from a hearing aid sales and service provider.
Exclusions include: non-prescription hearing aids, replacement batteries, recharging equipment, warranty or service costs for hearing aid maintenance, personal sound boosters, hearing magnifiers or amplifiers, sound amplifiers, or previously purchased hearing aids.
WW® online
Use your Benefit Bank to pay for online WW® (formerly Weight Watchers) membership costs.
Health care items
For members of our Saver No Rx, Orange, and Green plans, you get extra money on your Benefit Bank card to buy health care products. You’ll get either $125, $150, or $255—depending on your plan. You can shop for select over-the-counter health care products at specific retail locations or with our online partner, Medline. With Medline, you can make purchases online, by mail, or by phone—items will be delivered to your home at no extra cost.

Purchases must be made during the current calendar year. Money expires on December 31 of each year and does not roll over to the next year.
