Fallon Health announces a new location in Framingham to serve older adults in the MetroWest

Worcester, Mass., February 3, 2025—Fallon Health, a national leader in offering PACE (Program of All-Inclusive Care for the Elderly), is pleased to announce the opening of a new alternative care setting in Framingham. This location aims to bring care closer to home for older adults in the MetroWest region. Located at 405 Cochituate Road, it expands access to Fallon Health’s Summit ElderCare® program, providing a convenient space and location for participants to receive various services and portions of their care.

“One of the things that makes PACE so unique is our team-based approach to care. At our new location, participants and their families will have local access to many of our dedicated specialists, spending less time on the road and more time focused on their health and well-being,” said Kris Bostek, senior vice president of PACE programs. She noted that hundreds of individuals in the city and surrounding communities may be eligible for PACE.

The new space includes 3 exam rooms, lab services, a therapy room for physical, occupational, and other therapeutic services, a conference room for care planning and family meetings, a shower, laundry facilities, and dedicated office space. To ensure a comprehensive support system, Adult Day Health (ADH) services are provided through partnerships with like-minded local programs, including Dia Feliz Adult Day Health Center.
