Fallon Health offers new, affordable health care option for individuals and small groups in Bristol, Plymouth counties in 2025
Network providers include Signature Health, Beth Isreal Lahey Health, and Southcoast Health
Worcester, Mass., November 14, 2024— Beginning January 1, 2025, Fallon Health will expand the service area for its Community Care plans to include all of Bristol and Plymouth counties. Community Care offers eligible individuals, families, and small businesses an affordable option with competitive benefits and a robust network. Plans are offered through the Massachusetts Health Connector.
“As we enter the Health Connector Open Enrollment period that begins on November 1, Fallon Health is pleased to offer a new, affordable choice to help residents in southeastern Massachusetts stay healthy,” said Jennifer Lavigne, Community Care Senior Director. “Fallon Health is proud to offer a range products and services that deliver equitable, high-quality, coordinated care. And we’re thrilled that beginning in 2025, that will include Community Care for eligible individuals in this region.”
Community Care offers a $0 premium option for those who qualify* and member-focused benefits and services for all members. Healthy extras include free annual wellness visits and eye exams, a fitness reimbursement program, a program for expectant parents, a 24/7 nurse call line, and chronic care management and stop-smoking programs.
Fallon Health’s Community Care product will also offer access to a collection of providers designed with southeastern Massachusetts residents in mind including Signature Healthcare’s Brockton Hospital, Beth Isreal Lahey’s Beth Isreal Deaconess Hospital- Plymouth (BID Plymouth), and an integrated continuum of health services from Southcoast Health. New to the network in 2025 are Southcoast’s three hospitals— Charlton Memorial Hospital (Fall River), Tobey Hospital (Wareham), and St. Luke’s Hospital (New Bedford)—and Southcoast Physicians Group (SPG), a multi-specialty integrated network of providers in the region.
Community Care is also currently available in Berkshire, Middlesex, Suffolk, and Worcester counties, parts of Norfolk County and, also as of January 1, Hampden County.
