Fallon Health’s low-cost Community Care plans are available in Berkshire, Middlesex, Suffolk, and Worcester counties, and part of Norfolk County. And, effective January 1, 2025, the Community Care service area will include all of Bristol, Hampden, and Plymouth counties.

What do I get with Community Care?
Fallon Health’s low-cost Community Care plans include benefits and services to help you stay healthy. Plus, you’ll get extras like:
- Free annual eye exams
- The most flexible fitness benefit available in Massachusetts, reimbursing up to $150 annually for participating in healthy activities such as streaming fitness programs, Peloton subscriptions, weight loss programs, school and town sports, gym memberships, dance and fitness classes, home fitness equipment, aerobics, Pilates, yoga classes, ski lift tickets and much more
- Oh Baby!, a program that provides free prenatal vitamins, a convertible toddler car seat, electric breast pump and other “little extras” for expectant parents—all at no additional cost
- Care Connect, a free 24/7 nurse call line to help you get the care you need
- Telehealth. Members get 24/7 access to a national network of U.S. board-certified doctors. Our Community Care plans also cover telehealth visits with plan providers.
- Care management programs - for members with chronic conditions such as asthma, diabetes, and heart disease
- Sapphire365, a web-based tool to help you shop for the right health care providers for you based on cost and location
- Healthy Health Plan, a program that supports members (subscriber and spouse age 18 and older) in becoming, and staying, healthy. Simply fill out the health assessment, receive a personalized health report and then take advantage of all the tools available, including health coaching, to help you reach your health goals.

Get help paying for your health insurance
You can apply for our plans on the Massachusetts Health Connector. Depending on your income and your household size, you may qualify to get help paying for your health insurance. Don’t think you qualify for financial help? You might be surprised.
Looking for insurance for your small business?
If you have a small business with 1 to 50 qualifying employees and your business is located in the Community Care service area, you can purchase a Community Care plan for your employees and their families through the Massachusetts Health Connector.
Note: Small businesses with 6 to 50 employees can purchase a Community Care plan through the Massachusetts Health Connector (where they may be eligible to receive tax credits) or through Fallon Health.
