an alternative to living in a nursing home.
Your loved one is getting older, and their health care needs are changing but that doesn’t mean they should have to move to a nursing home.
With help from our PACE (Program of All-inclusive Care for the Elderly), your loved one can stay in their home for as long as possible.
PACE covers and provides:
- Visits with medical professionals, like doctors and specialists
- Prescription medications
- In-home services
- Therapies—physical, recreational, and more
- Meals and social activities at our adult day program
- Health insurance
- Care from professionals who specialize in geriatric care
- Transportation as needed
Where we provide care
All the care and services your loved one needs are covered and delivered to them, through 1 program.
Your loved one may receive care at our adult day program center, at their home, or a combination of both. Our Care Team will work with you and your loved one to create a care plan that is tailored to your loved one’s unique health needs.
Our program has expanded!
We’re now on the South Coast—in addition to many other areas of Massachusetts.
Program costs
Program participants have no copays, no coinsurance, and no deductibles—for care, medications, and services approved by the Care Team. The monthly premium may be covered by Medicare, Medicaid, and/or private pay (out of pocket).
To be eligible, program participants must:
- Be age 55 or older
- Meet the Social Security Title XVI disability standards—if between the ages of 55 and 64
- Live in the service area
- Need enough assistance to qualify for a nursing home level of care—but want to continue living at home
- Be able to safely live in the community, according to our Care Team
