Fallon Health’s Summit ElderCare® is
an alternative to nursing home care, providing support to
older adults who have advanced health care needs.

With our Summit ElderCare PACE (Program of All-Inclusive Care for the Elderly), participants can:

  • Live at home, safely and independently
  • Access health insurance, prescription drug coverage, medical care, social support, in-home services, and adult day health care—all in one convenient program
  • Receive care from, and have an individualized care plan developed by, a team of health care professionals that specializes in the care of aging adults
  • See doctors, receive on-site medical care, socialize, enjoy healthy meals, receive adult day health services, and take part in activities at one of our PACE Centers
  • Receive transportation to and from the PACE Center, and to off-site medical appointments

Peace of mind for caregivers

In addition to the above, we offer specialized care and activity space for participants with Alzheimer’s disease or dementia. And, we provide 24-hour access to nursing professionals for emergencies, a multi-lingual staff, monthly meetings for caregivers, and more.

To be eligible, your loved one must:

  • Be age 55 or older
  • Live in the service area
  • Need enough assistance to qualify for a nursing home level of care—but want to continue living at home
  • Be able to safely live in the community, according to our Care Team

All services must be provided by or authorized by the PACE Interdisciplinary Team (except emergency services). PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.
