Care and services
Summit ElderCare covers all services that are recommended and approved by the participant's Care Team. That includes, but is not limited to, the following:
- Prescription drug coverage
- Medication management
- Care management and support
- 24-hour telephone access to a member of the Care Team
- In-home nursing, rehabilitation, and other support services
- Physical, occupational, recreational, and other therapies
- Behavioral health services
- Specialized dementia care
- Nutrition evaluations
- Adult day health services
- Medical supplies and equipment
- Inpatient hospital care and discharge planning
- Specialty care, such as cardiology, ophthalmology, and podiatry
- Dental care
- Vision care, including eyeglasses
- Other services as prescribed by the Care Team
We provide these and other services to supplement the care participants receive from their families and other caregivers.
Close to home
Summit ElderCare PACE Centers provide participants with a place to receive their medical care and enjoy social and recreational activities in an adult day health setting that is close to where they live.

A Care Team to support participants and their families
The members of the Care Team—which may include a primary care provider, a nurse, a social worker, a physical therapist, a dietitian, and more—specialize in taking care of older adults.
